Health Choice Network, Inc. (HCNI) is a nation-wide collaboration among health centers and partners. Throughout its history, Health Choice Network, Inc. Has led the shift in cutting-edge technology that Health Center-Controlled Networks need to remain at the forefront of today’s technology-driven environment. CareQuest Institute provided Health Choice Network, Inc. with one grant of $7,378 in 2009.
This proposal is one of 10 selected finalists for the Community and Care Transformation RFP. Health Choice Network, Inc. is seeking funding to implement and test behavioral health and oral health integration practices in the Hawaii Island Community Health Center. The partner organizations will implement Pediatric Adverse Childhood Experiences and Related Life Events Screener (PEARLS) in the Epic electronic health record (EHR) which is utilized at Hawaii Island Community Health Center and at many Health Choice Newtork health centers. HCNI will collaborate with Hawaii Island Community Health Center to implement a referral pathway between the oral healthcare providers and behavioral healthcare providers. Following a pilot period, HCNI will
analyze program data to assess for an association between screening and referring for ACEs and positive oral health behaviors. Lastly, throughout the project, HCNI and Hawaii Island CHC will conduct patient surveys and focus groups with staff to determine patient satisfaction with the PEARLS intervention. As indicated in the proposal, and confirmed by Analytics & Data Insights, CareQuest Institute staff will be responsible for some data analysis, hosting focus groups, and submitting IRB materials for the project.
The community engagement plans for this proposal include the development stakeholder engagement panel, which will include patients and key community stakeholders, who will review all steps of project planning and implementation. The findings and benefits of this model will be shared with other FQHCs within HCN and at other health center-controlled networks.
The budget for this proposal is $125,000, which represents less than 1% of the organization's overall budget. About $40,000 is allocated for project staff time, and the remainder of the budget lines are delineated for program expenses like data management costs, training, a sub-grant for the Hawaii Island Community Health Center and marketing and communications costs.
