Support to the Pennsylvania Association of Community Health Centers for continuation of its role in the Strengthening the Oral Health Safety Net Initiative. Unique approaches include, but are not limited to: collaboration with key stakeholders across the state including PA Head Start, the PA Department of Public Works, the PAAAP, and the PA Coalition for Oral Health in the development of a statewide oral health plan, and continued opportunities for collective impact; incorporating a focus on chronic disease management into its newly formed annual integration summit; leveraging an EHR/EDR integration assessment performed during Year 2 to identify and replicate successful health record integration strategies.
