Support to the Arizona Association of Community Health Centers for continuation of its role in the Strengthening the Oral Health Safety Net Initiative. Unique approaches include, but are not limited to: collaboration with the California PCA and Western Clinicians Network in the education of clinicians on the integration of oral and behavioral health into Patient-Centered Medical Home efforts; partnership with EHR/EDR vendors to provide training and technical assistance to member centers on leveraging electronic records for integrated care; a comprehensive training for medical and dental assistants to support integrated care, focused on effective communication in the workplace, nutrition, oral health, and diabetes management, and preventive practices for pediatric patients.
