The South Carolina Research Foundation's (SCRF) is focused on improving the oral health status of vulnerable populations, specifically very young children. SCRF will pilot test a COHC model that moves the patient navigation component of Act 235 from a legislative concept to a sustainable public health response that is integrated into primary care and dental safety net systems to achieve the significant, measurable oral health outcomes designated by the Triple Aim. SCRF hopes to achieve the improved patient care experience of the Triple Aim, operationalized as an increase in the number of children who visit the dentist by age 1 year and have at least annual visits thereafter and an increase in the number of children aged 0-3 years who receive risk-based fluoride varnish from their primary care providers.
