Support to the South Carolina Research Foundation (SCRF) to implement a plan to improve the oral health status of vulnerable populations, specifically the early childhood populations for whom the SCRF will pilot test a COHC model that moves the patient navigation component of Act 235 from a legislative concept to a sustainable public health response, that is integrate into primary care and dental safety net systems, and will achieve significant, measurable oral health outcomes, as designed by the Triple Aim. During the next two years, SCRF hopes to achieve the ‘improved patient care experience’ component of the Triple Aim, operationalized as: increase in the number of children who visit the dentist by age 1 and have at least annual visits thereafter; and increase in the number of children aged 0-3 years who received risk-based fluoride varnish from their primary care providers.
