Banner Health is an Age-Friendly Healthcare System, recognized by the Institute for Healthcare Improvement, with locations in Arizona, Colorado, Nevada, and Wyoming.

In 2023, with partnership from the Delta Dental Foundation, Banner University Medical Center Phoenix (BUMCP) piloted the Hospital Oral Care and Proactive Education: HOPE Dental Program, which established an oral health nurse navigator and dental hygienist to facilitate oral health care for geriatric, hospitalized patients. HOPE has been expanded to all Banner Hospitals in AZ, CA, CO, NV, and WY

Through the Community Capacity and Voice Sub-Program, Banner Health seeks to advance the successes of the pilot by ensuring oral care is incorporated into the bedside care plan at BUMCP, creating personalized discharge plans and follow up for high-risk patients, providing intake and discharge surveys for each patient to improve program outcomes, and comparing Year 1 and Year 2 data to determine program efficacy and cost analysis.

This request is for $50,000, which is less than 1% of the organization’s overall budget. The grant funds will support staff time for Project PI and a Data Analyst, and $3,800 for travel to share program outcomes.
