This investment provides continued support to the State University of Iowa Foundation (Iowa) to continue to develop a Patient-Centered Dental Home (PCDH) model and quality measurement framework the promotes care integration within dentistry and between dentistry and the broader health home environment. The patient-centered medical home (PCMH) is an established model that aims to improve the organization and delivery of primary care in medicine. However, there currently is not a similar nationally recognized model for the delivery of dental care. As the delivery system is moving towards ACOs, prioritizing value over volume, this work is focused on understanding and systematizing the role of dental within this integrated framework. This framework can be used to incentivize dental providers to demonstrate improvement in dental care quality, oral health outcomes, and coordination with other facets of the health care system. Iowa has led a highly collaborative and engaged process with a diverse group of experts and professional organizations including the American Dental Association, DQA, NCQA, NASHP, ASTDD, the CDC, among others. With the National Advisory Committee, Iowa used a modified Delphi process to develop the definition of a PCDH and its associated characteristics. The focus of this grant renewal is to continue to build a measurable and definable PCDH model. Specifically, the grantee will develop a list of components that should be included in each PCDH characteristic, identify measure concepts for each component, and link existing quality metrics to those measure concepts. Iowa will also complete user testing of existing quality indicators and launch a public web tool, disseminate progress and results through meetings, presentations and explore publican opportunities, and develop a roadmap for PCDH implementation. $115,000 will be used to support personnel time, travel for meetings, a consultant to manage the Delphi methodology, and other administrative costs associated with the project’s implementation.
