New Jersey: This investment supports the American Academy of Pediatrics, New Jersey Chapter (NJAAP) to launch the NJ HeathHome 2.0 demonstration project aimed at fusing the relationship between primary care providers, dentists, and parents to optimize children's oral health. Working in partnership with New Jersey Dental Association and New Jersey Oral Health Coalition, NJAAP will aim to introduce and strengthen care coordination and provide an interprofessional approach to health care to achieve optimal health. This initiative will engage partners in a shared value strategy through a collective impact framework to build a scalable business model for national replication. The proposal will advance the systems change outcome of ensuring that oral health is integrated into all aspects of health care. It aligns with the Oral Health 2020 goal of eradicating dental disease in children and the target that by 2020 85% of children reach age five without a cavity. $50,000 would cover personnel and consultants time, travel and meeting expenses as well as indirect costs associated with operations.
