New Jersey: Support to the New Jersey Chapter, American Academy of Pediatrics (NJAAP) to serve as a facilitative leader of the New Jersey Oral Health Coalition's (NJOHC) Pediatric Health Home Subcommittee (PHHS) that advances the goals outlined in the New Jersey Oral Health Action Plan. Using a collective impact model, NJAAP will serve as a connector by engaging healthcare providers and grassroots organizations to join the network and work collaboratively, as well as by partnering with both traditional and non-traditional stakeholders to promote oral health awareness and develop mechanisms for consistent and comprehensive state-based data collection. The project will focus on supporting state-based champions and leaders, developing effective care models, as well as engaging advocacy community, care providers, and families. The proposal aligns with the Oral Health 2020 goal of eradicating dental disease in children and the target that by 2020, 85% of children reach age five without a cavity. $150,920 will support the personnel, organizational, and convening capacity necessary to expand outreach to public and private stakeholders at the state, county, and community levels.
