Massachusetts: Support to the Massachusetts League of Community Health Centers (MLCHC) to continue building capacity within Brockton, Dimock, Mashpee, Hilltown, and Lowell community health centers (CHCs) to deliver oral health services in primary care. As part of this project, MLCHC will provide technical assistance (TA) and coaching to help these five CHCs implement key primary care oral health functions – assessment, screening exams, preventive interventions, care coordination and referral management, quality improvement and metrics, as instructed in the Qualis Oral Health Delivery Framework. The proposed project will leverage MLCHC’s expertise in quality improvement and Patient-Centered Medical Home (PCMH) practice transformation, and build on their commitment to help CHCs deliver high-quality, patient-centered, whole-person care to vulnerable patients. Specific outcomes to be addressed by this project are the integration of oral health into health care and improvement of consumer-focused care delivery. $300,800 will support the staffing, consultant services, and TA capacity needed to implement the project. The proposal aligns with the Oral Health 2020 goal of integrating oral health into person-centered healthcare.
