Kansas: This investment supports the Kansas Association for the Medically Underserved (KAMU) to implement and field test a set of tools under developed by Qualis Health to integrate oral health into Patient-Centered Medical Home (PCMH) models in Community Health Centers. The integration includes health information technology (HIT) and electronic health records with a focus on enhanced procedures and work flow for referrals and shared information. KAMU will engage two community health centers (CHCs) that do not provide oral health care services onsite. This proposed work intends to prepare CHCs to provide oral health preventive services in the context of their primary care medical home, to better manage referrals to dental care, and to ensure high-quality, well-coordinated care. $98,950 will support personnel time, travel and meeting expenses, and a contract with Qualis Health to support the implementation of its toolkit within Kansas CHCs. This proposal aligns with the Oral Health 2020 environmental support of including oral health in interprofessional education and coordinated care models.
