Survey — Oral Health Care Coordination and Clinical Information Systems in Federally Qualified Health Centers  

Thank you for responding to our invitation to participate. Participation in this survey will quantify the issues and experiences affecting the sharing of patient health information, both within the FQHC and with outside health care organizations.  

Participation is open to one medical provider (i.e., physician, nurse practitioner, or registered nurse) and one dental provider (i.e., dentist or dental hygienist) from your FQHC (not to exceed two provider participants per organization). 

Participating providers must provide clinical services at your organization but can also serve in administrative positions (i.e., department directors).

The survey will take 15-20 minutes to complete.  Providers who successfully complete the survey will receive a $25.00 gift card incentive. Incentives will be sent electronically within 14 business days of survey completion.
